Upon checking-in, please alert the front desk staff member of any changes to your contact and billing information and be prepared to provide a copy of your new insurance card at the time of your appointment.
You can also update your insurance, personal and medical information from within “My Health Record” on this client portal. As you may have several physicians, this information can change without the treating physician being aware of these changes. We ask that you login prior to any scheduled appointment to keep this information current.
